Registration 2013-2014 Summary

The following forms must be completed for all students:
- Enrollment Form
- Emergency Form
- Immunization Records (Please have your physician fax them to 818 505-7997)
If you intend on requesting scholarship assistance, you must also complete this scholarship application (even returning students).
If you do not require scholarship assistance, you need to sign the VTHS Tuition Agreement and set up your FACTS Management account for the coming year:
- For returning students: Complete the FACTS Management Renewal Agreement.
- For new students: Open a FACTS Management account at the VTHS – FACTS Online Portal. or complete a written FACTS Management Tuition Agreement (available in the school office or at the )
